This application will be required to be made via “former – 3”. For this, you are supposed to attach a scanned copy of the documents that are necessary such as:
- A copy of your PAN Card that is self-attested.
- Id proof of the supposed directors.
- Residential addresses proof of your directors and
- Passport size photograph of all proposed directors.
This form shall be attested by any practising charted accountant or a company secretary or cost accountant. And submit all this to the MCA portal.

- RUN- Reserve Unique Name form is for name approval of the name.
- INC – 12 – This form is for the issuance of a license as under the section 8 Company’s Act 2013.
